HIPAA Notice of Privacy Practices

Platinum Medical Club ("PMC," "we," "us") is committed to protecting the privacy of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Alaska law. This notice describes how medical information about you may be used and disclosed, and how you can get access to this information.

How we may use and disclose your health information

• Treatment: To provide, coordinate, and manage your care — including with specialists, laboratories, and pharmacies involved in your treatment.
• Payment: To prepare superbills and documentation you request for submission to your insurance carrier or health sharing plan.
• Health care operations: For quality assessment, staff training, and the business operations of our practice.
• As required by law: When disclosure is required by federal, state, or local law, including public health reporting and court orders.

Your rights

You have the right to:

• Inspect and obtain a copy of your medical record.
• Request an amendment to your record.
• Request restrictions on certain uses and disclosures.
• Request confidential communications (for example, by a different phone number or address).
• Receive an accounting of disclosures.
• Obtain a paper copy of this notice at any time.

We will never sell your PHI or use it for marketing or fundraising without your written authorization. Any use or disclosure not described in this notice requires your written permission, which you may revoke at any time.

Complaints

If you believe your privacy rights have been violated, contact us using the information below. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

SMS / Text Message Privacy Policy

Consent and what we send

By providing your mobile phone number and opting in, you consent to receive SMS text messages from Platinum Medical Club. Messages may include appointment confirmations and reminders, replies to your questions, and other care-related communication. Message frequency varies. Message and data rates may apply depending on your mobile carrier and plan.

No sharing of mobile information

Your mobile phone number, your decision to opt in, and the content of text message conversations that relate to your care are treated as part of your protected health information. We do not sell, rent, or share your mobile information or opt-in status with third parties or affiliates for marketing or promotional purposes. Text messages are exchanged only between you and authorized PMC staff.

Opting out

You may opt out of SMS messages at any time by replying STOP to any message. After you opt out, you will receive one final message confirming you have been unsubscribed. To resubscribe at any time, reply START or contact our office. For help, reply HELP or call (907) 600-5200.

Carriers and delivery

We use third-party messaging services to deliver text messages. Carriers are not liable for delayed or undelivered messages. If you have concerns about receiving sensitive information by text, let us know and we will use a different method to communicate with you.
